AI + VA: the no-guessing model

AI Should Not Guess. It Should Know When to Validate.

Bexara uses AI to process insurance information and execute defined workflows. When information is unclear, contradictory, incomplete, or requires human judgment, the workflow can stop for review instead of allowing an unsupported assumption to continue. That validation is carried out by the Bexara VA team.

Who does what

Clear roles for AI, reviewers and the platform.

AI executes

  • Retrieves documents and information
  • Extracts policy and coverage details
  • Classifies emails and documents
  • Compares policy information
  • Performs defined checks
  • Prepares updates and downstream actions

AI validates

  • Checks extracted information against available source documents
  • Applies configured business rules
  • Identifies missing information
  • Flags contradictions and exceptions
  • Determines when additional review is needed

VA — Virtual Assistant / Human Reviewer

Provided by the Bexara team
  • Reviews flagged exceptions
  • Checks the original source
  • Corrects extracted information when necessary
  • Requests missing information or escalates complex cases
  • Approves actions where the workflow requires approval

Bexara coordinates the workflow

  • Routes work to the appropriate capability
  • Maintains workflow state
  • Prevents unresolved issues from progressing when configured rules require a hold
  • Records decisions, changes and review outcomes

VA validation is handled by the Bexara team.

When a workflow pauses for human validation, Bexara's own VA team reviews the flagged items against the source documents, confirms or corrects them, and releases the workflow — so your staff don't have to work the review queue. Your agency keeps the approval steps your SOPs require.

Example

A coverage limit the AI got wrong — and what happens next.

Watch an extracted value conflict with another source. The AMS update pauses, a VA checks the original documents, and the workflow resumes only after validation. Use Play, Pause, Back, Step and Replay.

Bexara does not treat a plausible AI-generated answer as verified information.

Statuses

Every value says how it was checked.

Statuses describe what has actually happened to a piece of information. A value is never marked verified just because a model produced it confidently. Each workflow uses only the statuses that apply to it.

  • AI ExtractedProduced by an AI model. Treated as a candidate value, not a fact.
  • Source VerifiedConfirmed against the original document or system of record.
  • Rules ValidatedPasses the configured business rules for this workflow.
  • Human ReviewedA reviewer has checked the value against its source.
  • ApprovedCleared for the downstream action the workflow requires.
  • Requires ReviewWaiting for a person. Configured downstream steps hold.
  • UnresolvedMissing or contradictory and not yet decided.

When validation is required

When AI encounters uncertainty, Bexara brings in human validation.

Instead of allowing an unsupported assumption to move downstream. Each workflow defines which situations require a hold, and what can proceed automatically.

Unclear

A value is hard to read, ambiguous or below a confidence threshold.

Contradictory

Two sources disagree — for example, a declarations page and the binder in the AMS.

Incomplete

A required field is missing from every available source.

Judgment required

The decision affects coverage, the client relationship or a regulated action.

Configured, not hard-coded. Your agency decides which checks run, which conditions require a VA, and which actions need approval. Holds are enforced by the workflow engine, so a flagged item cannot quietly continue downstream.
